Based in the beautiful village of Bartestree, approximately 5 miles east of Hereford city, St Michael’s Hospice has been providing the highest quality care, to the local community for over 35 years.

As a Hospice at Home Nurse you will use your nursing skills and knowledge, complemented by the education, support and mentoring we will provide, to ensure all patient’s end-of-life care needs are addressed. You will work with multiple agencies, advising, explaining and acting as a patient advocate in conversations with GPs, paramedics and other healthcare providers. This multi-disciplinary collaboration, alongside excellent communications and teamwork within the charity, means your patients will have continuity of care and be able to focus on quality time with their family.   

You will have the time, support and training to gain a more in-depth understanding of patients’ needs and how to address them. Your work is varied, always focusing on the care and support each family needs for their loved one to spend their final days at home.  

Visiting patients in their own homes, you will be providing expert palliative care alongside emotional and psychological support for the whole family. You will be building a rapport over weeks, months or even years so that symptoms are controlled, hospital admissions prevented and families able to spend quality time together, at home.
