 Key focus of the role includes:

• Promote healthy weight and maintenance of healthy weight

• Support people to be more physically active

• Support people to improve and maintain good mental wellbeing

• Bring individuals into more effective contact with mainstream health improvement and other local services

• Increase numbers accessing lifestyle support

• Increase numbers accessing Primary Care/Health & Social Care where appropriate Key responsibilities

• Work with partners to promote the service, encourage referrals from professionals working within and outside the Primary Care Networks and from the public.

• Develop trusting relationships by giving people time and focus on ‘what matters to me’. Take a holistic approach, based on the person’s priorities and the wider determinants of health.

• To work with clients to assess their individual needs and co-produce a tailored personal health plan with agreed goals and outcomes.

• Deliver both one to one and group-based support where required, setting up new groups where required and facilitating social and peer support.

• Support people to develop knowledge and skills to support themselves to maintain their own health and wellbeing, including signposting onto appropriate self-help tools and information.

• Managing and prioritising referrals and caseload

• Accurate and timely recording of information and outcomes based on Service requirements

• Work as part of a local network of lifestyle advisors to seek feedback and continually improve the service. July 2021 Weight Management Lifestyle Advisor Key Tasks Referrals

• Promoting the lifestyle service within the local Primary Care Network, neighbourhood teams, wider partners and the public.

• Build relationships with key staff in GP practices within the local Primary Care Network (PCN), attending relevant meetings, becoming part of the wider network team, giving information and feedback on the lifestyle service.

• Be proactive in developing strong links with all local agencies to encourage referrals.

• Work in partnership with all local agencies to raise awareness of the lifestyle service and how partnership working can reduce pressure on statutory services, improve health outcomes and enable a holistic approach to care.

• Provide referral agencies with regular updates about the lifestyle service, including how to access information to encourage appropriate referrals.

• Seek regular feedback about the quality of service and impact of the lifestyle service on referral agencies.

• Be proactive in encouraging self-referrals and connecting with all local communities, particularly those communities that statutory agencies may find hard to reach. Provide personalised support

• Meet people on a one-to-one and group basis. Give people time to tell their stories and focus on ‘what matters to me’. Build trust with the person, providing non-judgemental support, respecting diversity and lifestyle choices. Work from a strength-based approach focusing on a person’s assets.

• Be a friendly source of information about wellbeing and prevention approaches.

• Help people identify the wider issues that impact on their health and wellbeing, such as debt, poor housing, being unemployed, loneliness and caring responsibilities. Work closely with the social prescribing service to seek support when required. • Work with the person, their families and carers and consider how they can all be supported through lifestyle and behaviour change support.

• Work with individuals to co-produce a simple personalised support plan – based on the person’s priorities, interests, values and motivations – including what they can expect from the groups, activities and services they are being connected to and what the person can do for themselves to improve their health and wellbeing.
