Job overview
We are looking for a proactive, forward-thinking pharmacy technician to join our experienced lead Clinical Pharmacist and newly recruited Clinical Pharmacist in the Wyre Forest Network of Independent Practices PCN.
We have a vacancy for a Full Time Pharmacy Technician at Agenda for Change band 5(depending on qualifications and relevant experience), with terms and conditions based on Agenda for Change.
Main duties of the job
You will work as part of a multidisciplinary team and will be expected to develop a patient-facing role over time. You will take responsibility for areas of chronic disease management within the network practices and undertake clinical medication reviews to proactively manage patients with complex polypharmacy.
You will be required to:
· assist with the management of prescription requests and working under the supervision of the pharmacist making (pre-agreed) medicine management changes at the point of ordering. This could include, but is not limited to
· Brand/generic changes;
· Synchronizing medications;
· Removing unused medication from repeats;
· Checking for excessive ordering, particularly of prn medicines.
· supporting GPs with medication queries from patients: working within competencies to manage queries and redirect to appropriate clinicians if necessary
· ‘triage’ clinic letters and discharges – working within competencies to reconcile medication changes, or redirect to the clinical pharmacist or GP if necessary
- Supporting the Pharmacists to increase uptake of electronic Repeat Dispensing by targeting very specific patient groups – reviewing their medication and, with support manage the transition and ongoing housekeeping when medications are stopped or changed etc.
- Support general practice staff with prescription and medication queries
About us
Wyre Forest Network of Independent Practices grew out of a mutual respect and vision of how primary care could work collaboratively without merger and was formalised in February 2018, when the group of 5 signed an MoU to work in a mutually supportive manner. This pre-dated the Network Enhanced Service (by some 18 months), and meant that we had already developed a strong ethos of joint working, collaboration and trust. We have had a lead Clinical Pharmacist in post since October 2019, and now wish to expand the Network Pharmacy team with the recruitment of a Pharmacist and Pharmacy Technician.

We are looking for a proactive, forward-thinking pharmacy technician to join our experienced pharmacy team.
We have a vacancy for a Pharmacy Technician to work 5 days per week at Agenda for Change band 5 (depending on qualifications and relevant experience), with terms and conditions based on Agenda for Change.
You will work as part of a multidisciplinary team. You will work within the 5 member practices dealing with mutually agreed areas for that practice and other specific pharmacy requirements for each practice under the Lead Clinical Pharmacist, and working with the existing practice Medicines teams and the newly appointed Network Clinical Pharmacist
You will be required to:
- Support general practice staff with prescription and medication queries
- Support the repeat prescription systems
- Deal with acute prescription queries
- Conduct medicines reconciliation on transfer of care within pre-defined areas
- Support the implementation of systems for safer prescribing
Person Specification
Qualifications
Essential
- GPhC Registered Pharmacy Technician
- BTEC/NVQ level 3 or equivalent in pharmacy services
- Accredited Checking Technician
- Enrolled in, undertaking or qualified from, an approved training pathway. For example, the Primary Care Pharmacy Educational Pathway (PCPEP) or Medicines Optimisation in Care Homes (MOCH)
