Course Outline:
An educational and interactive course, providing individuals with an understanding of a personalised care approach, and how this is implemented and embedded in primary care. It will provide tools and techniques in communication, health coaching, managing conflict, increasing motivation, reducing health inequalities, and identifying barriers, to ensure that high quality patient care is provided.
An overview of skills and competencies required, and NHS contractual requirements, demonstrating how roles work together across all organisations, and sharing good practice.
Signposting to further resources, information, and encouraging continuous professional development. Inspiring individuals with the confidence to put what they have learnt into practice.
Speaker for the session: Holly Johnson
Holly is a busy mum of 2 who loves to do anything creative! She started working for the NHS in 2015 as an Administrator and then qualified as an HR Administrator.
After further training in Leadership and Management, Holly moved on to Operations Supervisor with SW Healthcare and then into her current role PCN Operations Manager with Droitwich, Ombersley & the Rurals PCN.
She is also the Non-Clinical Ambassador for Hereford and Worcestershire with the Training Hub, Health Inequalities Lead for Worcestershire, and a qualified coach for the system.
Day One - Learning Outcomes:
- What care co-ordination is in the context of primary care.
- What benefits care coordination has for patients.
- What keys skills you need to become a Care Coordinator.
- What effective communication is and what the barriers can be.
- How to write a case study.
- What coaching is and how it can be used in the Care Coordinator role.
- Being able to confidently use a coaching model.
- What a person-centered approach is and why we use it.
- Different tools and techniques used in coaching that can be used in the Care Coordinator role.
Day Two – Learning Outcomes:
- What patient activation is and how it is measured.
- Identification of barriers, resistance to change and techniques on how to increase motivation.
- Being able to deal with challenging patients.
- The importance of reducing health inequalities in healthcare
- An understanding of what services are in the NHS and where the care coordinator role fits in.
- The competencies needed for care coordinators.
- What training is needed and where to access it.
- What networks are available to find information and share learning.
