Job Summary:

We are seeking a proactive and compassionate Care Coordinator to join Worcester Primary Care Network (PCN) on a maternity cover basis. The postholder will support patients with personalised care planning, coordinate services across health and social care, and promote continuity of care within the PCN. The role requires close working with multidisciplinary teams, patients, carers, and other professionals to improve patient experience and outcomes.

This is an exciting opportunity to work in a brand new role within Droitwich, Ombersley and the Rurals (DOR) PCN. The role will give you a unique chance to work as a member of DOR PCN team, whilst also building links with a range of other local services and be part of a county wide Work and Health coach network. We will particularly welcome applicants with lived experience and those with strong connections to Droitwich, Ombersley, Knightwick, Great Witley and Chaddesley Corbett areas.

The successful candidate will be part of the Workwell programme and will work with people from a range of backgrounds to help them return to work. They will work closely with participants within the local community to assess the barriers they experience in returning to work and provide support and coaching along with health and employment advice to overcome these barriers and return to work.

This role would suit someone with previous experience of coaching and mentoring, clinical experience is not required and training will be provided for the successful candidate.

This role is based within a GP practice in the city of Worcester working alongside a multidisciplinary GP practice team, including other pharmacists, GPs, nurses etc. The working day is variable and includes designated practice focussed work and at scale work for the PCN including working on localised contracts such as Clinical Excellence Improvement framework. The role will also be part of a larger PCN pharmacy team with free staff on site parking.

There are many opportunities to develop as an Independent prescriber (if not already held) as well as opportunities to work with the wider PCN team on projects focussed on improving health inequalities.

This role presents an exciting opportunity to directly assist in the management, improvement and growth of Worcester City Primary Care Network delivering insights and Analysis to Primary Care.

The Data Analyst will work with the Primary Care Network and Partners, providing insights and intelligence to ensure that the Primary Care Network have the means to provide better patient care. We aim to offer and deliver excellent, safe, co-ordinated, high quality patient analysis. Continuously learning, evolving and implementing new models of care, to create inclusion and reduce inequalities and improve outcomes.

This is an exciting role and an opportunity to make a unique and measurable impact on patient care, also enhancing the effectiveness and efficiency of Integrated services. The successful candidate will be responsible for building high trust relationships with local Health and Care Teams as well as Practice based Clinicians. The postholder will develop reporting, dashboards, and analysis to support the Primary Care Network and improve Patient Care.
