We are looking for a Salaried GP at our innovative GP practice now run by the GP federation, where we champion the doctor patient relationship & continuity of care at practice level, underpinned by the expertise of the federation & at scale services. Barnt Green Surgery has the following working hours available and is looking for a friendly hard-working receptionist to join their team supporting both the reception desk and telephone access for our patients. There will also be the need to support the team with holiday and sickness cover.

Monday, Tuesday and Wednesday 1 – 7pm and Thursday 1-5pm

We have an exciting opportunity for a second non-clinical Wellbeing Care Coordinator to join our Health & Wellbeing Team in North & West Primary Care Network.

The Wellbeing Care Coordinator (WBCC) is the central care coordination role with the N&W PCN Wellbeing Team. They will act as the first point of contact for all new referrals to the Wellbeing Team and are the single point of access for any referral issues, advice and queries to all external partners within General Practice and community teams.

The successful candidates will be caring, dedicated, reliable, and person-focused and enjoy working with a wide range of people. They will have good written and verbal communication skills and strong organisational and time management skills. They will be highly motivated and proactive with a flexible attitude, keen to work and learn as part of a team and committed to providing people, their families and carers with high quality support.

Job Title: Social Prescriber

Hours: Full-Time (Monday – Friday, occasional Saturdays if required)

Position: Care Co-ordinator
Location: Vale of Evesham PCN
Hours: 20-30 hours per week
Salary: Band 3-4 (depending on experience)

This job description is generic and there will be variation to the position dependent upon the locality where the job is. All of the locality roles include care planning; attending MDT meetings; reviewing of medication; reviewing of patients following a hospital discharge; assisting with avoiding unplanned admissions; working closely with GP’s; recording information on the EMIS clinical system; being a patients first point of care as their named care coordinator. However, there will be variants across the locality which will be made clear prior to appointment.

Care Home Practitioner roles will also be considered as a merged role with the care coordinator role in various localities based upon the applicant’s previous experience and skill set. These localities and merged roles will be made clear prior to appointment.

The successful candidate will work under the clinical supervision of the Frailty Team ACPs and GPs, visiting patients in residential and nursing homes, as well as patients in the community to assess a variety of health conditions.  

You will be working within the PCN in a general practice environment based at Maple View Medical Practice. The core hours will be Monday – Friday 0800 – 1600 hours, which aids a good work life balance. On occasion you may be required to work additional hours to suit the needs of the team.

The South & West Herefordshire Primary Care Network (PCN) is delighted to be recruiting for a Health & Wellbeing Coach to join our team.

The Health & Wellbeing Coach will partner with other members of the PCN team to support local initiatives, but as an experienced practitioner they will also influence and guide improvement at a county level.
