Pharmacy First for Care Navigators: Supporting Effective Referrals to the Pharmacy First Service Education Session
Taking place on Thursday, 22 January online on the LMS from 13:00 - 14:00.
Increasing the number of appropriate GP referrals to Pharmacy First supports a neighbourhood approach to delivering integrated, proactive healthcare at a local level. It enhances patient access and can reduce pressure on general practice through optimal use of community pharmacy pathways. This session aims to equip care navigators in general practice with the knowledge, confidence, and practical tools needed to make safe and effective referrals to Pharmacy First services.
Please note places are very limited and will be allocated on a first-come, first-served basis, restricted to one care navigator per practice.
Learning Objectives:
By the end of the session, participants will be able to:
- Describe the purpose, scope, and benefits of the Pharmacy First service
- Confidently explain the referral process and patient journey
- Identify which patients and presenting conditions are suitable for Pharmacy First referral
- Distinguish the seven Pharmacy First clinical pathways from general minor ailments
- Use consistent communication approaches to encourage patient acceptance of pharmacy referral
- Apply learning to real-world scenarios within their practice setting
