Social Prescriber

Main Duties of the Role
As a Social Prescriber, you will work directly with people to understand their personal circumstances and help them access the right mix of support for their health and wellbeing. This could mean connecting someone to community groups, arranging practical advice around housing or finances, or encouraging healthier lifestyle choices through activities such as exercise, arts or nature-based programmes.
You’ll listen carefully, build trusting relationships, and create personalised support plans that give individuals the tools to manage their own wellbeing and live more independently. Collaboration will be a key part of the role – you’ll work closely with GP practices, the wider PCN team, voluntary sector partners and local services to ensure people get joined-up, practical support.
This role is ideal for someone who enjoys making a tangible difference every day, thrives on building connections, and is passionate about tackling inequalities in health and wellbeing across the community.
