Based in the beautiful village of Bartestree, approximately 5 miles east of Hereford city, St Michael’s Hospice has been providing the highest quality care, free of charge, to the local community for over 35 years.  

This is an exciting opportunity to join the friendly and multidisciplinary Hospice at Home team. You will be responsible for assessing, planning and delivering a high quality palliative and end of life care to patients and families within their home or care home. The role provides an opportunity to provide support to those most in need and enables patients to stay in their preferred place of care.  

You will undertake and oversee the comprehensive holistic assessment, planning implementation and evaluation of the needs of patients on each shift. You will deliver generalist and complex nursing care interventions that is responsive to the individual patient’s requirements. You must be a registered nurse, and have experience and knowledge of palliative care.
