Key Responsibilities:

• Provide personalised care and wellbeing coaching to a cohort of patients.

• Work closely with GPs and multidisciplinary teams to develop and implement patient care plans.

• Participate in multidisciplinary team meetings to discuss patient progress.

• Engage with community resources and initiatives to promote healthy lifestyles.

• Manage a caseload independently, ensuring patient outcomes are met.

• Support quality improvement projects across the PCN.

 

What We’re Looking For:

• A good understanding of health coaching, social prescribing, and non-clinical support services.

• Experience working within a multidisciplinary team, ideally in a healthcare setting.

• Excellent communication skills, with the ability to build rapport and work effectively with a diverse patient population.

• Ability to manage your workload independently and think on your feet.

• Flexibility to work across different locations and some weekends.

Essential Qualifications and Skills:

• GCSEs in English and Maths (or equivalent).

• A relevant coaching qualification.

• IT literate, with experience using databases.

 

Desirable Experience:

• Experience working with individuals experiencing low-level mental health issues.

• Knowledge of health and nutrition, smoking cessation, or drug and alcohol brief interventions.

• Experience working in GP practices or similar healthcare settings.
