Safeguarding Care Co-Ordinator

Main duties of the job
The Safeguarding Care Co-Ordinator post provides a rewarding opportunity to work alongside lead GPs and Practice teams to develop efficient processes and systems at the very heart of general practice. They support member practices to:
Provide central co-ordination for safeguarding information an communication to help GPs plan care for vulnerable patients.
Maintain consistent protocols for safeguarding administration, document handling, record management and communication with partner organisations.
Support multi-disciplinary team working - communicating between teams who have a joint role in looking after vulnerable patients; the Primary Care Network team, Multi-Agency Safeguarding Hub and the CCG.
