Job overview
We are looking for a newly registered, proactive, forward-thinking pharmacist to join our experienced Pharmacist Team in the Wyre Forest Network of Independent Practices PCN. This is a great opportunity for a recently registered Pharmacist who is looking to pursue a career within General Practice as you will be working within a very supportive environment aiming to develop the Pharmacist role in General Practice, alongside an already experienced team.
We have a vacancy for a Full Time Clinical Pharmacist at Agenda for Change band 7. You will be required to complete the mandatory NHSE CPPE pathway upon successful completion of this, we may then look to support you through the Independent Prescribing pathway.
Main duties of the job
You will work as part of a multidisciplinary team and will be expected to develop a patient-facing role over time. You will take responsibility for areas of chronic disease management within the network practices and undertake clinical medication reviews to proactively manage patients with complex polypharmacy.
You will be required to:
- Support general practice staff with prescription and medication queries
- Oversee the repeat prescription system
- Deal with acute prescription requests
- Conduct medicines reconciliation on transfer of care
- Implement systems for safer prescribing
- Provide clinical medicines advice
About us
Wyre Forest Network of Independent Practices grew out of a mutual respect and vision of how primary care could work collaboratively without merger and was formalised in February 2018. This pre-dated the Network Enhanced Service (by some 18 months), and meant that we had already developed a strong ethos of joint working, collaboration and trust. We have had a lead Clinical Pharmacist in post since October 2019, and also employ 2 full time pharmacists and 3 full time pharmacy technicians. We now wish to expand the Network Pharmacy team with the recruitment of another full time Pharmacist.

We are looking for a proactive, forward-thinking pharmacist who wishes to pursue a career in General Practice to join our experienced pharmacy team.
We have a vacancy for a Clinical Pharmacist to work 5 days per week at Agenda for Change band 7.
You will work as part of a multidisciplinary team and will be expected to develop a patient-facing role over time. You will work within the 4 member practices and undertake clinical medication reviews to proactively manage patients with complex polypharmacy, and other specific pharmacy requirements for each practice under the Lead Clinical Pharmacist, and working with the existing practice Medicines teams and Network Pharmacists and Pharmacy Technicians.
You will be required to:
