Job Purpose
SW Healthcare is looking to recruit a Nurse Associate to work across the four GP practices in Malvern Town PCN . Your role may be focused on a particular cohort of patients for example one of the roles is in our newly developed care home team or a more general role working with people of all ages.
The post holder will have completed their Nurse Associate training and be registered with the NMC able to work independently under the leadership of a practice nurse.
As a Nurse Associate you will contribute to most aspects of general practice nursing providing hands - on person centred care, working as part of a multidisciplinary team your main duties will cover promoting health and preventing ill health, delivering and monitoring care, contributing to the delivery of integrated care and improving the safety and quality of care.
The role offers an excellent next step for Health Care Assistants looking for career progression who want to move from task and protocol-based care to delivering more holistic and patient centred care. Beyond this, the role provides a good progression route into graduate level nursing.
JOB RESPONSIBILITIES
Promoting health and preventing ill health
Provide continuing care to patients in Nursing and Residential Care Homes within Malvern Town PCN
Monitoring and support of the management of patients with long-term conditions / chronic diseases, using a holistic patient centred approach
Support the co-ordination and management of patient cohorts to ensure they receive the appropriate level of care and that indicators such as QOF are achieved
Working in teams
Chaperone patients when required
Assist in the training and development of other staff, in particular Healthcare Assistants and Trainee Nursing Associates in line with professional competencies for the role
Communicate effectively with other members of the primary health care team across the Network.
Attend relevant meetings as directed.
Providing and monitoring care
Provide continuing care across the primary care population, assigned cohort of the Networks population or to specific GP practice populations
Clinical duties in line with competency e.g. Suture/clip removal, basic and complex wound care, ECG. Phlebotomy, Urinalysis

The post-holder will:

Work with the Lead GP(s) to achieve standards of quality, performance standards, without compromising levels of patient healthcare.
Contribute towards the development and implementation of new standards, policies and procedures that are/will be required of GP Practices now and in the future (as directed by NHS/ DoH/, new legislation etc.)
Communication
Be able to effectively communicate at all levels of the organisation to a variety of health professionals, users and carers, independent and voluntary sector to provide the best outcomes for users of the services.
Provide interface between hospital, primary, community and social settings, participate where appropriate in clinical meetings relating to patient care and outcomes
Communicate effectively with patients and carers
Be able to keep accurate contemporaneous documentation, both written and computerised, inpatient records and will need to be familiar with EMIS Web.
Recognise peoples needs for alternative methods of communication and respond accordingly
Clinical Governance
To participate and operate within the clinical governance framework for the organisation at all times, incorporating service users and carers, audit, guidelines and risk management.
To actively participate in the practices, becoming familiar with and abiding by its plans, policies and procedures.
This job description may be reviewed in the light of changing organisational and service needs. Any changes will be fully discussed with the post holder. The post holder may also be required to carry out other work appropriate to the grade of the post.
LOCATION
The role will mostly be based in Malvern Town member practices and will require some travel amongst PCN member practices and business meeting locations in Worcestershire. The post holder should be flexible in their working hours which may include some evening and weekend work.
