Job responsibilities

The PCN Nurse will work as part of the Worcester City Primary Care Network multidisciplinary team to deliver high-quality, patient-centred nursing care across a range of primary care services. The post holder will undertake a variety of clinical duties, including wound care, cervical screening, immunisations, venepuncture, health promotion, and the delivery of preventative health interventions. They will support the management of patients with long-term conditions, contribute to population health initiatives, and work proactively to improve health outcomes and reduce health inequalities within the local community.

The successful candidate will work collaboratively with GP practices, PCN colleagues and wider healthcare professionals to ensure safe, effective and timely care for patients. They will support the delivery of national and local programmes, including NHS screening and vaccination initiatives, maintain accurate clinical records, and contribute to service improvement activities.
