Patient Care Liaison Officer

FULL TIME POSITION (37 hours per week) 

An opportunity has arisen for a highly motivated, innovative, enthusiastic person to join our clinical team.  You should have an interest in advocating the needs of our patients and will have the responsibility of providing a comprehensive service to ensure their care needs are met.  You will provide care and interventions centred on individual patient needs to achieve positive outcomes and will liaise with community partners to ensure continuity in care.

Part of your role will involve working with patients experiencing memory loss and early stages of dementia.  Memory assessment experience is not essential however some knowledge of Dementia care will be required in this post.

Main duties of the job

This is a new developing role within our practice and the successful candidate will have excellent interpersonal and communication skills, be organised, patient and empathetic. You will have experience of working in health, social care or other support roles including direct contact with people, families or carers.

We are a semi-rural dispensing practice of approximately 9,850 patients based in the historic market town of Evesham, on the edge of the Cotswolds, and with excellent transport links to Cheltenham, Stratford-upon-Avon, Birmingham, Oxford and London.  We pride ourselves on promoting a traditional ethos of General Practice within a modern healthcare environment.

The Practice currently has 3 Partners, 3 Salaried GPs and an excellent supporting team which includes a Clinical Pharmacist, Paramedic, Physician Associate and an experienced, well-established Nursing team.
