hey will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures. As part of the Wyre Forest Health Partnership, the role will be based at Kidderminster Medical Centre working with the neighbourhood team. Our aim is a more integrated holistic care approach. The post older will be part of a dedicated visiting and frailty resource supported by a visiting GP.
