Main duties of the job

As part of the main duties, the successful candidate will be expected to include the following tasks:

Reduce inappropriate polypharmacy and wasteful prescribing

Reconcile medicines following hospital discharge

Contribute to reductions in medicine to hospital admissions and readmissions

Oversee and support the dispensary functions of the Practice(s)

Oversee repeat prescribing reauthorization process

Manage Patient's and healthcare professionals' queries
