Clinical Pharmacist

We are looking for an experienced Clinical Pharmacist confident with :
- Clinical medication Reviews
- Managing medication queries from patients, community pharmacies and fellow healthcare professionals.
- Ensure safe, effective and rational use of medication.
- Medicine Optimisations Audits.
- Monitoring patients on high-risk medication.
- Hospital discharge reconciliation and liaising with hospital and community pharmacies to ensure continuity of supply.
- Repeat prescription process management.
- Long term conditions clinics, experience of management of diabetes would be highly desirable.
- Prescribing by pharmacist with Independent Prescribing status.
- Implementing drug alerts and withdrawals e.g. MHRA alerts
